Our Home Transition Services are designed to provide the support and guidance you need to make this shift as smooth and stress-free as possible. We work closely with you and your healthcare team to ensure your home is ready, providing personalized assistance with medical equipment setup, home safety assessments, medication management, and more. Our goal is to help you regain independence and confidence as you adjust to life at home, with care and support every step of the way.

• Evaluation of patient while in rehab facility or hospital

• Home evaluation with caregiver/designated person present while patient still in rehab

• Extensive evaluation summary with all recommendations to implement in preparation for a safe discharge to home

• Connection to a general contractor for any home adjustments needed
